Chevelle Brudey M.D.

Dr. Brudey completed medical training at Tulane University in Louisiana. Early in medical school, she became excited about pursuing combined training in internal medicine and psychiatry. She completed her residency in internal medicine and psychiatry as well as fellowship in geriatric psychiatry at Emory University in Atlanta, Georgia. Dr. Brudey has a deep appreciation for the importance of the patient’s unique narrative. She has often been inspired by patients’ resilience, creativity and innovation in solving complex problems and in navigating shared decisions between patient and physician. Before moving to Bent Tree Health Professionals, Dr Brudey served as an assistant professor at the University of Texas Southwestern where she worked as an internist and psychiatrist and led two integrated behavioral health clinics. Her interests outside of medicine include her family, cooking, reading, and theater.

Dr. Mascarenhas received his M.B., B.S., and M.D. Degrees from the University of Bombay, India. He served a rotating internship at Goa Medical College, and a general psychiatry residency at both the University of West Indies and the University of Alabama in Birmingham. He is Board Certified in General Psychiatry. He was an instructor in Clinical Psychiatry at the University of Alabama, a staff psychiatrist at the Veterans Administration Medical Center in Dallas, Texas, Assistant Professor of Clinical Psychiatry at Southwestern Medical School in Dallas, Texas, and Medical Director Substance Abuse and Dual Diagnosis for St. Paul Medical Center, also in Dallas, Texas. He is currently serving as Medical Director of Psychiatric Services at Methodist Richardson Regional Medical Center. He has been in private practice since 1982, and has a vast knowledge of both causes and effects of drug and alcohol addiction disorders and general psychiatric disorders. He also provides other services, such as electroconvulsive therapy, and rTMS.

Dr. Steinfink was born in Texas, and grew up in Austin. He attended the University of Texas and then went to Southwestern Medical School in Dallas. He completed his residency in adult and child psychiatry at Parkland Memorial Hospital and Children's Medical Center of Dallas. He has practiced adult, adolescent, and child psychiatry in Dallas since 1978. Currently he works in our Dallas location, and treats adults for general psychiatric conditions. Since 2012, Dr. Steinfink has been involved in providing Transcranial Magnetic Stimulation, an innovative treatment for depression and other conditions. He attended the Harvard course in rTMS, and is the Texas Clinical Director of TMS Neurosolutions.

For the past 25 years, Dr. Johnson has provided comprehensive mental health care to the people of North and East Texas. Dr. Johnson completed his undergraduate education at Rice University prior to attending the University of Texas Medical Branch in Galveston, Texas. Following medical school, Dr. Johnson completed a pediatric internship at The University of Texas Health Science Center in Houston, a residency in psychiatry and behavioral health at The University of Texas Medical Branch, and a fellowship in Child and Adolescent Psychiatry. Dr. Johnson now specializes in the treatment of Attention Deficit/Hyperactivity Disorder, Autism Spectrum, Behavioral Disorders, Substance Use Disorders, Mental Disorders due to primary medical illnesses, Delusional Disorders, Disorders of Adult personality and behavior, and Mood and Anxiety Disorders. In joining the clinical professionals of Bent Tree Psychiatric Associates, Dr. Johnson hopes to enhance the mental health care of his patients.

Dr. Donachie graduated from the University of Texas Medical School at Houston in 1979, and completed her psychiatric training there in 1983. She spent 4 years as an Assistant Professor of Psychiatry at the University of Texas Medical School at Houston and was the Medical Director of the adult psychiatric unit at Hermann Hospital during that time. She began her practice in the Dallas/Richardson/Plano area in 1984. She has served as the Medical Director of Psychiatric Services at Texas Health Presbyterian Hospital-Plano served as Medical Director from 1993 until 2018 when she became the Chief Medical Officer for TMS Neurosolutions and was Medical Director of Psychiatry at Methodist Richardson Medical Center until 2010. She also has worked as a medical director in a managed care setting. She is one of the founding physicians of Bent Tree Psychiatric Associates, and is the Chief Medical Officer of rTMS.

Dr. Crumpacker received his medical degree form Cornell University Medical College in New York City before completing a residency in general psychiatry at the University of Texas Southwestern Medical Center at Dallas. He then completed a two-year fellowship in geriatric psychiatry also at UT Southwestern. A diplomat of the American Board of Psyhiatry and Neurology, double-boarded in both adult and geriatric psychiatry, Dr. Crumpacker served as the Assistant Chief of Psychiatry at Baylor University Medical Center at Dallas from 2005-2009.

Dr. Crumpacker's research centers on risk factors associated with Alzheimer's disease as well as diagnosing and treating Pseudo Bulbar Affect.

As a nationally sought lecturer on depression, Pseudo Bulbar Affect, and Alzheimer's disease, he also publishes professionally and advocates for patients' rights. He has been recognized by both D Magazine and Texas Monthly as an outstanding physician.

She received her BA in Biology from the University of Kansas in 1989, and her MD from the University of Texas Health Science Center in Houston, Texas in 1994. In 1998, she completed her residency at the University of Texas Medical Branch in Galveston, Texas in general psychiatry, and her fellowship in forensic psychiatry at UT Southwestern in 1999. Sara Jaquay received her Bachelor of Science in Nursing and her Master of Science in Nursing from The University of Texas at Arlington. She is board certified by the American Nurses Credentialing Center as a Psychiatric Mental Health Nurse Practitioner. Sara is also recognized by the Texas Board of Nursing as an Advanced Practice Nurse. She specializes in adolescent, adult and geriatric psychiatry.

Sara began her nursing career at the age of 17 and worked in the long term care environment caring for patients with Alzheimer’s disease. After a couple of years she was promoted to Assistant Director of Nursing and then Director of Nursing where she was responsible for managing and overseeing all areas within the nursing department. Her primary focus was on promoting patient-specific care with an emphasis on improving patient outcomes. She then moved on to work in the acute hospital setting where she specialized in trauma and orthopedic care. During this time she cared for patients with co-occurring psychiatric illnesses. She identified a need for providers with specialized training in diagnosing and treating psychiatric illnesses. Sara is passionate about delivering patient-specific care and working with her patients to improve symptoms and quality of life.

In her spare time, Sara enjoys spending time with her husband and daughter. She actively participates in her church and volunteers in the children’s and youth ministry. Sara also enjoys volunteering at local homeless shelters.

Roland is currently the Nurse Practitioner for Dr. Laura Slaughter at our McKinney office. In 2000, he received his Bachelor of Science degree in Chemistry from the University of Doula, in Cameroon, and his Master's of Science in Chemistry from the New Jersey Institute of Technology in 2005. He has both a Bachelor's and Master's degree of Science in Nursing, which he received from the University of Texas at Arlington in 2011 and 2017. From November 2014 until 2017, he was a staff RN for Green Oaks Hospital, dealing with adults with psychiatric disorders, including but not limited to, psychosis, Bipolar disorders, chemical dependency, and schizophrenia. He was also a staff RN for Medical City of Dallas and North Hills Hospital from 2012-2014.

Mr. Shahin was born in Oklahoma City where he resided until he was 6 years old.  He then moved to Amman, Jordan where he resided until age 13, subsequently attending boarding school in Limassol, Cyprus.  He returned to the Oklahoma City at the age of 16 where he obtained his BA degree in Business Management at Southern Nazarene University followed by his BS  as a Physician Assistant at the University of Oklahoma. Dr. Saira Dar completed her medical education and residency at Regions Hospital in Minnesota in 2005. Then she joined a private practice in Kokomo, Indiana, where she maintained a full-time practice, managed several nursing homes, and started Kokomo’s first opioid dependency treatment program. This opioid dependency treatment program was followed by several Kokomo practices after she moved to Texas in 2008 and joined the University of North Texas Health Science Center in Fort Worth. After completing a fellowship in academic medicine, Dr. Dar started a dual career in academics and clinical practice in 2010. She designed, developed, and implemented clinical courses at the Texas College of Osteopathic Medicine, and presented at national and international conferences on behalf of TCOM. Dr. Dar worked as a medical director while maintaining a full-time practice. She completed a Master of Business Administration from the University of Massachusetts, Amherst, in 2015, and will be completing a Master of Education in the Health Professions from Johns Hopkins University in the summer of 2018.

Dr. Dar finished fellowship training in anti-aging and integrative medicine in 2018, and also obtained certificates in personalized and genomic medicine. Her passion is to provide care that is based on the principles of personalized and integrative medicine and improved health-spans. She focuses on medicine, nutrition, and genomics to cater to each individual patient’s needs. Believing that each of us is unique, Dr. Dar develops a customized care plan for one patient at a time. She is active in the community and on social media.

Dr. Khan, who received his medical degree from King Edward Medical College at the University of the Punjab in Pakistan, is a board-certified neurologist with more than 10 years experience.
After completing his residency at Kaleida Health Buffalo General Medical Center, Dr. Khan completed a Clinical Neurophysiology Fellowship – Epilepsy Track at Duke University. He was then recruited to Wentworth Douglas Hospital as a General Neurologist and Prospective Director of the Epilepsy Unit.
We are happy that Dr. Khan chose to join the Smart Neuro Health team to provide comprehensive, integrated health care to every patient who walks through our doors.

Addison Zaya is a native to Midland, Texas. She began her education at Midland College, where she was a student athlete. After graduating with her Associates of Applied Sciences, she decided to continue her studies and athletic journey at Hardin Simmons University, where she obtained her Bachelor of Science in Nursing in 2015. She then attended The University of Tennessee Health Science Center in Memphis, where she obtained a Doctorate of Nursing Practice in 2018. Addison simultaneously specialized in both Primary Healthcare and Specialty Psychiatric Healthcare, studying the design of integrative medicine. Addison is double board certified by The American Association of Nurse Practitioners as a Family Nurse Practitioner, and The American Nurses Credentialing Center as a Psychiatric Mental Health Nurse Practitioner. Due to her integrative background, Addison takes a comprehensive and individualized approach to care, with emphasis on preventative interventions. She is able to provide primary care and specialty psychiatric services for children, adolescents, adults, and geriatric patients.

In her spare time Addison is an exercise enthusiast, continues to enjoy playing volleyball and other sports. She’s often found spending time with friends, family, and her dog, Maggie.

Olayemi (Yemi) Ononogbu received her Bachelor of Science in Nursing from Texas Tech University Health Sciences Center, in May 2009. Yemi also received her Master of Science in Nursing from Texas Woman’s University, in May 2013. She is board certified by the American Academy of Nurse Practitioners, and recognized by the Texas Board of Nursing as an Advanced Practice Nurse.

Starting off her career as a Registered Nurse in 2009, Yemi worked in the general intensive care unit in Lufkin, TX after which she moved back to Dallas for graduate school. She worked at Texas Woman’s University as a graduate assistant for the duration of her studies there. Yemi has since then, worked in the home health settings as well as private practice settings as a Nurse Practitioner. Yemi is passionate about helping people and has found this calling specifically in the healthcare field. Yemi includes her patients in determining their care as this promotes compliance to treatment plan, which ultimately increases quality of life.

Yemi enjoys spending time with her family and friends, giving back to the community, cooking, reading, and would love to travel the world.
